{"id":205505,"date":"2020-04-27T12:07:39","date_gmt":"2020-04-27T12:07:39","guid":{"rendered":"https:\/\/www.watchdog.md\/?p=205505"},"modified":"2023-03-15T20:02:24","modified_gmt":"2023-03-15T20:02:24","slug":"moldova-healthcare-system-readiness-and-covid-19-pandemic-evaluation","status":"publish","type":"post","link":"https:\/\/watchdog.md\/en\/news\/205505\/moldova-healthcare-system-readiness-and-covid-19-pandemic-evaluation\/","title":{"rendered":"MOLDOVA: Healthcare System readiness and COVID-19 pandemic response evaluation"},"content":{"rendered":"<p>This report was\r\nprepared based on the publicly available information on April 10, 2020, is\r\nvalid and relevant in accordance with this timeline. <\/p>\r\n\r\n\r\n\r\n<p>The\r\npreparedness stage for this pandemic was covered by the fragmented actions and\r\ndid not ensure a fast and full response to this emergency outbreak. <\/p>\r\n\r\n\r\n\r\n<p>The\r\ncollective responsibility approach, the lack of defined timelines, the lack of\r\ndesignated resources creates difficulties in the crisis management process and\r\ngenerates multiple undesired outcomes.<\/p>\r\n\r\n\r\n\r\n<p>The health authorities\r\nneed to be prepared to communicate with their populaces about\r\nhealth issues and protective actions that they can take. <\/p>\r\n\r\n\r\n\r\n<p>The Country Pandemic\r\nPlan as a pre-event tool is required for a better execution of preparedness\r\nstage and fast response to any disease outbreak.<\/p>\r\n\r\n\r\n\r\n<p>The quality of the response to this pandemic outbreak was influenced on a large scale by the low level of awareness of the pandemic emergency on the top level of management of the healthcare sector. <\/p>\r\n\r\n\r\n\r\n<p>The delay in the elaboration of the response country pandemic plan leaded to the delay in the decision-making process and generated several delays in execution of many important action items.<\/p>\r\n\r\n\r\n\r\n<p>All these actions limited the power of control over pandemic spread in some geographies and increased the risk for population, and specifically for the healthcare workers, police, custom staff.<\/p>\r\n\r\n\r\n\r\n<p>An\r\nepidemiologic investigation and the accompanying response are the backbone of\r\noutbreak control. <\/p>\r\n\r\n\r\n\r\n<p>Technological\r\nadvances are needed to modernize our diagnostic capabilities to become faster\r\nand nimbler at the onset of outbreaks, particularly around novel pathogens.\r\nDiagnostic tools are required in settings beyond centralized laboratories. <\/p>\r\n\r\n\r\n\r\n<p>To\r\nadequately prepare for and respond to outbreaks, health facilities would need\r\nto increase their capacity for large-scale isolation of patients with highly\r\ntransmissible respiratory diseases. The biological safety and security require holistic\r\napproach and proper resources.<\/p>\r\n\r\n\r\n\r\n<p>It is recommended to\r\nincrease the community engagement through\r\ndialogue, power sharing, collaborative decision making, and combined actions\r\namong a community. Doing this, its leaders can strengthen readiness, response, and\r\nrecovery in the case of pandemic\r\noutbreak. <\/p>\r\n\r\n\r\n\r\n<p>The communication with the public, partners, and\r\nintermediaries as well as between key organizational stakeholders continues to\r\nbe an important area for strengthening preparedness and response. Without strong, accurate communication\r\nefforts, no amount of planning, intervention, or response is likely to be\r\nhighly effective in the response to a pandemic.<\/p>\r\n\r\n\r\n\r\n<p>The communication and the establishment of the trusted lines of communication between the\r\ncommunity and public health is a vital component of effective public health\r\nresponse. <\/p>\r\n\r\n\r\n\r\n<p>The application of the\r\ntravel restrictions, movement\r\nrestrictions, quarantine, and social distancing, is efficient in\r\nconditions of the strong adherence to the rule and require the high level of\r\ntrust between community and state institutions. <\/p>\r\n\r\n\r\n\r\n<p>In the event of a rapidly moving pandemic, the\r\nGovernment would need to be able to quickly deploy sufficient\r\nresources in order to protect the population and reduce the fatality rate.<\/p>\r\n\r\n\r\n\r\n<p>Moldova\r\nneeds support to develop the core public\r\nhealth capacities to prevent, detect, prepare for, and respond to any disease\r\noutbreaks. <\/p>\r\n\r\n\r\n\r\n<p>With this purpose, the\r\ncountry needs a robust national process and a system\r\nto enable the collection, analysis, and dissemination of surveillance data, an essential tool for identifying and\r\nunderstanding patterns and drivers behind a wide range of health threats. <\/p>\r\n\r\n\r\n\r\n<p>The\r\nprocesses and the systems used to conduct risk\r\ncommunication and national surveillance can be multipurpose, serving everyday\r\nhealth priorities, but then tapped, and modified or expanded, if necessary,\r\nduring emergencies. <\/p>\r\n\r\n\r\n\r\n<p><strong>The estimated\r\ndemand for resources, in order to support the actions mentioned in Article 3\r\nare presented in the annex 1. The focus was concentrated to the most urgent and\r\nimportant items, such as PPE insurance for HCP, renovation of the infectious\r\ndiseases department in the district hospitals, and upgrade of the Intensive\r\nCare Units (ICU), by increasing the number of the ICU beds across the country. <\/strong><\/p>\r\n\r\n\r\n\r\n<p><strong>The needs for\r\nPPE are calculated based on the daily needs for 35000 HCP, in order to cover\r\n100 days of pandemic outbreak. <\/strong><\/p>\r\n\r\n\r\n\r\n<p><strong>The need for\r\nrenovation of the departments for infectious diseases as a primary measure to\r\nbe prepared for the next wave is calculated for 36 hospitals.<\/strong><\/p>\r\n\r\n\r\n\r\n<p><strong>There is a gap\r\nof 800 ICU beds in Moldova and urgent need to cover this gap. In the annex 1 is\r\nmentioned the full list of equipment required to support this action. The\r\nMcKenzie company mentioned in their report about pandemic outbreak, as one of\r\nmandatory criteria for the country reopening \u2013 the presence of &gt;5 ICU beds\r\nfor 10 000 population. Currently in Moldova there are only 500 ICU beds for 2,6\r\nM people.
